Your health information is personal, and we are committed to preserving the confidentiality of your health information created or maintained at our Medical Center. Your health information is also very important to our ability to provide you with quality care, and to comply with certain laws. This Notice applies to all records about your care that occurs at our facility, whether the records are created by Medical Center personnel or by your physician.

Your physician and/or health plan may have different policies and a different notice regarding your health information that is created in the physician's office and/or at the offices of the health plan.

We Are Legally Required to Safeguard Your Protected Health Information.

We are required by the Health Information Portability and Accountability Act (HIPAA) a federal law as well as state laws to: Maintain the privacy of your confidential health information, also known as "protected health information" or "PHI"; and provide you with this Notice of our duties and privacy practices regarding your health information.

We reserve the right to revise the Notice and make the revision apply to your health information that we created or received prior to the effective date of this revision.
